We’re located at 721 American Avenue, Neuroscience Care Suite 108, Waukesha, WI 53188, within ProHealth Waukesha Memorial Hospital. From the West: Follow Madison Street east toward downtown Waukesha, turn right onto Fairview Avenue, at the first cross street turn left onto Lawndale Avenue, then turn right at the sign for ProHealth Waukesha Memorial Hospital East Entry. From the East: Follow Madison Street west from downtown Waukesha, turn left onto Fairmont Avenue, at the first cross street turn right onto Lawndale Avenue and make an immediate left at the sign for ProHealth Waukesha Memorial Hospital East Entry. Convenient valet parking is available at the east entry, or you may park in the adjacent garage.
