Goodman Campbell Brain and Spine accepts most insurance plans, including the ones listed below. However, it is your responsibility to contact your insurance company to confirm coverage and understand your responsibility for payment. Referrals
HMO and POS programs often require a referral from your primary care physician (PCP). Please ensure your PCP sends your referral prior to your appointment. Due to managed care restrictions, we are unable to treat some patients without a referral.

Preauthorization/certification
Most insurance plans require some type of advanced notification before any tests, procedure or hospitalizations. Please check your insurance card regarding preauthorization/certification. It is the patient’s responsibility to advise our office of your plan’s requirements.

Paying your bill

Goodman Campbell accepts all major credit cards, as well as the Care Credit Medical credit card. For more information on Care Credit, please call patient financial services at 317-396-1300 ext. 1198.

Insurance claims
You will be asked to sign a release of information and an assignment of benefits allowing Goodman Campbell to bill your insurer directly and to provide the information needed to process your claim. You are responsible for copayments and any remaining balance at the time of your visit.

Medicare
Goodman Campbell will submit claims to Medicare for our patients as well as to any supplemental insurance carriers.

Medicaid
Please bring your current Medicaid card to each visit. If you are in a managed care program, a referral from your primary care provider will also be required.

Workers’ compensation
You will be asked for the name of your workers’ comp carrier, address, claim number, contact person and phone number when scheduling your appointment. If your employer has not authorized your visit, full payment will be required at the time of service.
