You, the patient, are at the center of our work, and we are honored to serve you. As our patient, you can expect exceptional skill, every technical advance and a deep commitment to your health. Our team will work closely with you to make sure you receive excellent and compassionate care.

What to Bring With You

In preparation for your first appointment, we have outlined a few instructions to ensure we make the most of your appointment with us. Please take time to review these items, as they are important and relevant to your care.

  • Patient Forms
    Patient forms must be completed prior to your appointment. Here you’ll find patient forms you can print ahead of time, along with instructions on how to complete them.
  • Insurance Cards
    Please bring your insurance card each time you visit the office. It is an important guide to your insurance company’s precertification, copayment and claims requirements.
  • X-rays and Test Results
    To allow our providers to fully assess your condition, we need access to your diagnostic images. If we are unable to obtain your images electronically, you may be asked to bring a CD containing your X-rays, CT, MRI, etc.

If you are unable to keep your appointment, please let us know 48 hours in advance by calling (317) 396-1300.


Release of information

Federal regulations require Goodman Campbell Brain and Spine to obtain a signed release—you can print one here—before releasing any medical information about you or obtaining your medical records from another provider.
