You have several choices if you want to refer a patient to Goodman Campbell Brain and Spine: call us, fax us or submit a request online.
Referring physicians can complete this online form. We use TLS encryption to transmit this information to our office. We'll call your patient and set up an appointment, according to your preferences.
Then, the next time you need to make a referral to us, our system will remember the last NPI number you entered and fill in the contact information section of the form. You'll just need to complete the patient information. Please complete an online form for each patient you wish to refer to us. If you would like us to contact your office to set up the patient's appointment, please note this either in the "Diagnosis" or "Prior Testing" section. Otherwise, we will contact the patient directly to set up the appointment.
We will at least need your contact information as well as information about your patient - contact information, diagnosis or signs and symptoms. The more information you can provide on this form, the better able we will be to schedule your patient with the best provider for his or her condition.

Most patients come to us by referral from other physicians, after an initial work up that often includes CT and MRI scans, lab work and a physical examination. This information is sent to our office and reviewed by one of our physicians prior to the patient's appointment.
Referring physicians can be assured they will receive a timely and complete consultation report and are encouraged to call the Goodman Campbell provider with any questions.
