INSURANCE & PAYMENT INFORMATION

Your care is of the utmost importance to us, including your in-office experience. Because insurance benefits, requirements, and restrictions can be complicated and confusing, we've compiled this list of insurance guidelines to help create a stress-free, positive visit. Insurance Plans

Goodman Campbell accepts most major insurance plans, including the ones below. However, it is your responsibility to contact your insurance company to confirm coverage and understand what your responsibility is for payment. Even though your insurance plan may be listed below, it is not a guarantee of coverage. Please read the other sections on this page for more information about insurance coverage.

Insurance Card

It is important to bring your insurance card each time you visit the office, as well as a photo ID. The card is an important guide to your insurance company's pre-certification, copayment, and claims requirements. Please advise the receptionist of any changes to your contact information, your marital status, insurance, or primary care physician. 

Referrals

HMO, PPO, and POS programs often require a referral from your primary care physician (PCP). You can bring a referral form to your appointment, or it may be faxed by your PCP. Due to managed care restrictions, we are unable to treat some patients without a referral. 

Pre-Authorization/Certification

Before any tests, procedures, or hospitalizations, please check your insurance card regarding pre-authorization/certification. Most insurance plans require some type of advance notification. It is considered each patient's responsibility to advise our office of your plan's requirements.

Pre-surgery Financial Responsibility

We will verify insurance coverage prior to surgery. During this process, a Patient Financial Counselor will be in contact to determine the amount of financial responsibility you are likely to incur. It is our policy to require patients to pay their portion of the surgery prior to receiving services from their surgeon. 

Insurance Claims

Before or after your appointment, we will ask you to sign a release of information and an assignment of benefits allowing us to bill your insurer directly and to provide the information needed to process your claim. You will be responsible for copayments and any remaining balance after your carrier's payment. If we do not receive correspondence from your insurance company within 60 days from the date your claim is filed, you will receive a statement from Goodman Campbell. Patients with an open balance will receive a monthly statement until the account is resolved by your carrier and/or personal payments. All patient balances should be paid in full within three months after insurance payment resolution. Any exceptions to this policy, including patients without health insurance, should be discussed with our Patient Financial Counselor.


Forms of Payment

Medicare

Goodman Campbell will submit claims to Medicare for our patients as well as to any supplemental insurance carriers. Be sure to provide complete supplemental insurance information on your registration paperwork.

Medicaid

Please bring your current Medicaid card to each visit. If you are in a managed care program, a referral from your primary medical provider (PMP) will also be required.

Workers' Compensation

You will be asked for the name of your work comp carrier, address, claim number, contact person, and phone number when scheduling your appointment. If your employer has not authorized your visit, full payment will be required at the time of service.

Claim forms

Disability, family medical leave, or loss-of-time claim forms will be completed by Goodman Campbell for a nominal charge. Complete your portion and deliver or mail paperwork to our office accompanied by a check or money order.

Make a Payment

We have partnered with PNC Bank for an easy and secure way for you to submit a payment online. Please click 'Make a Payment' to go to a new, secure page and enter payment information.

Goodman Campbell accepts all major credit cards. Once you have submitted your payment, you will receive a confirmation e-mail. The payment will appear as “Goodman Campbell Brain and Spine” on your credit card billing statement.
